AMERICAN LEGION ONLINE REGISTRATION (AGES 16-19)

Please fill in all player and parent information.  Indicate the anticipated league based on the players age, final team placement will be determined by the Legion coaching staff.  Mark how you plan to volunteer.  By submitting you are agreeing to the waiver at the bottom of the page.  After submitting information below you will then be taken to a payment page.  Registration deadline:  DECEMBER 31, 2012.  Neenah Baseball is required to commit to number of teams at each age group by 1/15/2012. Please register early, registration does not guarantee a roster spot.

The Legion commitment deposit is a non refundable fee that commits you to the Legion program for
the upcoming season. The deposit will be applied to your $275 legion fees. The remaining $100 is due by 6/1/2012.

As the parent/guardian of the player named above, I give my approval to participate in any and all league activities.  I assume all risk and hazards incidental to participation, and do hereby waive, release, and absolve Neenah Baseball, Inc., its organizers, sponsors, participants, and volunteers of any claim arising out of injury to my child, except to the extent and in the amount covered by accidental liability insurance.  I agree to return the uniform and other equipment issued to my child in good condition.  I will furnish a certified birth certificate of the above named candidate to league officials upon request.
